MATCHING GIFTS
Would you like to double or triple your donation to MHCO?  Would you prefer your donation to be deducted from your payroll, monthly, quarterly, or annually, reported as your annual gift?  Matching gifts are a great way to maximize your personal contribution to MHCO while introducing your employer to a worthy charity, which you support.

1. Contact your employer’s Human Resource Department or Corporate Giving Office.
2. Please note that each company has its own guidelines for employees, spouses, retirees, and widows/widowers.
3. Request a Matching Form from your company.
4. Complete and submit the form as instructed.
5. Your company or MHCO will verify eligibility and then, if eligible, issues a matching gift contribution to MHCO.
6. Completed forms can be sent per the instructions of your employer to:
